In keeping with our mission to provide the most effective therapy available we need to have a good understanding your health history and current concerns.  To accomplish this, we require all new clients to complete an initial health intake form.

Our last order of business before getting started is your review and signature on the Massage Therapy Practice Policies and Informed Consent.  This form provides a brief overview of what you can expect from our office and how we work toward your good health and wellbeing.
